Por essas razões, o estudo da morfologia e microbiota dessa região em particular podem trazer importantes considerações terapêuticas, principalmente ao que tange ao limite apical de trabalho, variável que influencia diretamente o sucesso da terapia endodôntica.
O método radiográfico tem sido o mais utilizado para determinação do comprimento de trabalho desde sua introdução na Odontologia. Porém, é um método com inúmeras limitações como distorções nas imagens, superposição de estruturas anatômicas e a impossibilidade de determinar a exata localização da junção cementodentinária.
A deposição fisiológica de dentina secundária e cemento causam mudanças na localização da constricção apical em relação ao ápice radiográfico, levando a mensurações inadequadas do comprimento de trabalho e consequentemente, problemas na instrumentação do sistema de canais radiculares. Assim, buscando maior confiabilidade e precisão na determinação da localização do forame apical, surgiram as técnicas eletrônicas, alvos constantes de estudos e avanços na Endodontia.
A ideia inicial de utilizar corrente elétrica para estabelecer o comprimento de trabalho surgiu em 1918, mas o desenvolvimento dos localizadores apicais eletrônicos (LAEs) só foi possível pois a resistência elétrica entre o ligamento periodontal e a mucosa oral é constante (6,5 KΩ).
As duas primeiras gerações de LAEs eram sensíveis ao conteúdo do canal radicular e aos irrigantes utilizados durante o tratamento, visando corrigir esse problema, em 1989 surgiram os aparelhos de terceira geração.
Baseados no método da diferença entre impedâncias, estes LAEs não eram afetados pelos eletrólitos e seus resultados clínicos eram aceitáveis.
Em 1991, surgiu o “ratio method”, aumentando a precisão das medições feitas com os LAEs, surgem assim os aparelhos de quarta geração, utilizados atualmente.
Desde então, o emprego dos LAEs se difundiu na prática clínica, tanto de especialistas quanto de clínicos gerais.
A cada dia, um aparelho novo surge no mercado, porém a literatura científica não acompanha a velocidade de tais lançamentos.
A principal dificuldade enfrentada pelos clínicos e pelos pesquisadores é a falta de uma informação clara da parte dos fabricantes, em relação a quais estruturas anatômicas seus aparelhos são capazes de localizar. Atualmente, carecemos de estudos que comprovem a segurança e precisão desses novos aparelhos.
O presente estudo tem por finalidade comparar a acurácia de três localizadores apicais na localização do forame apical: Root ZX II (J Morita, Califórnia, Estados Unidos da América), Root ZX Mini (J Morita, Califórnia, Estados Unidos da América) e RomiApex A-15 (Romidan UK LTD, Kiryat-Ono, Israel).
Para atingir o objetivo, 32 dentes humanos permanentes unirradiculares, tiveram seu comprimento real determinado pela inserção de um instrumento endodôntico #10, até que sua ponta fosse visualizada na borda mais coronal do forame apical, com auxílio de um microscópio estereoscópico (magnificação de 16x).
Os dentes randomizados foram imersos em alginato e as medições eletrônicas foram feitas em duplicata com cada LAE. As médias das mensurações foram calculadas e comparadas com o comprimento real dos dentes. A diferença média entre esses valores foi de 0,50 mm para o ZX II, 0,45 mm para o ZX Mini e 0,50 mm para o RA A-15.
Com uma tolerância de 0,5 mm, os valores de precisão dos LAEs foram de 62,5%, 56,2% e 50%, respectivamente. Para uma tolerância de 1,0 mm, os valores foram de 87,5%, 96,87% e 87,5%, respectivamente.
Em conclusão, os LAEs testados não diferem quanto a precisão em detectar o FA, os três aparelhos apontam em média, 0,49 mm aquém deste ponto.

During the last 3-4 months, we’ve all experienced an unusual situation. In most parts of the world, The COVID-19 pandemic put us all in lockdown, a situation that has had a powerful impact on our businesses. Dental clinics were or still are closed for any treatment, other than emergencies.

Being in the dental equipment and materials supply business, providing dental solutions, the dream of having the best quarter or even the best year ever has turned into a nightmare of missed quotas. What should we do?

If we’re looking for tactics for filling the gaps between the hoped-for and the current amounts of sales, we are in the wrong place. This article is not about tactics, but rather about changing the mindset.

In a nutshell, our advice to you: look at your business and portfolio from a different angle, have a new perspective.

Here are 3 ideas, fresh from Forumtec’s marketing team.

1. Revise your portfolio

Yes, this is easy, yet a tricky one. This unique situation of the COVID-19 that we are all facing allows us to enter exploration mode. Whether you provide apex locators, dental implants, or other equipment, this is a great time to learn about our customer’s needs. A good service and product provider’s job is to anticipate what a consumer will need before they need it.  Take as an example the cleaning and disinfecting challenge of the dental office. They have always done a superior job of infection control and sterilization, but COVID-19 brings about new issues and warrants even further measures to protect patients and staff. They are committed to following the most stringent recommended guidelines. These days, the dental practice might ask for more cleaning, disinfecting, or sterilizing equipment or materials.

But here is the thing, is it the perfect set of messages for the COVID-19 days? Is it what your customers really need today? Let’s re-think. This is exactly what we did at Forumtec. It’s not that other attributes have become irrelevant. Only that certain ones are even more important these days. It’s a strange time for all of us, no matter where you are or the brand you represent. We’re all figuring it out as we go. A comforting thought, though, is that even though this rocky time in our collective history, one thing is always clear and reliable when it comes to your customers: creativity.

So you can either add products to your portfolio, renew your sales and marketing tools, or refresh your selling messages. Whatever you choose, be creative and look forward to the new normal days because of the new normal = new opportunities.

Clinicians’ top priority should be the health and safety of their patients and staff.  Dentists should follow standard contact and airborne precautions including the appropriate use of personal protective equipment and hand hygiene practices (1). Extraoral imaging such as panoramic radiography or a CBCT should be used to avoid the gag reflex or a cough that may occur during intraoral imaging (1). When extraoral imaging is not an available option, the use of intraoral imaging should be minimized, and in a personal note, if root canal treatment is performed, intraoral imaging should not be performed for length measurements. The available scientific evidence proves that apex locators perform better for the working length determination (2).

A rubber dam should be used throughout the procedure to reduce aerosols and minimization of the use of high-speed handpieces and ultrasonic instruments are recommended.

It is mandatory for health care providers to keep up to date with the rules and restrictions since the rampant spread of COVID-19 increases the likelihood that dentists will encounter carriers of the virus.
